About the Scholarship

The Jamie Bax Memorial Scholarship is a financial aid program dedicated to supporting U.S. students diagnosed with Cystic Fibrosis (CF) who are pursuing higher education. Established in memory of Jamie Bax, the scholarship empowers students living with CF by alleviating financial challenges and encouraging academic and professional success. The Jamie Bax Memorial Scholarship provides financial support for students with Cystic Fibrosis enrolled in accredited U.S. institutions. Recipients are recognized for their academic excellence, personal achievements, and leadership.

Degree Levels Supported:

• Bachelor

• Non-Degree

• Short Training / Vocational programs

Available Subjects:

• All subjects at accredited colleges, universities, junior colleges, or trade schools

Benefits Include:

• Financial awards: Three scholarships per year:

  • $5,000 (1 award)

  • $3,000 (1 award)

  • $2,000 (1 award)

• Funds may cover tuition, fees, books, housing, and meal plans

• Recognition of academic and personal achievements

• Opportunity to reapply annually (up to four awards in total)

• Reduced financial stress, allowing focus on education and health

Eligible Nationalities:

• U.S. citizens only

Eligibility Criteria:

• Confirmed diagnosis of Cystic Fibrosis (physician documentation required)

• Proof of U.S. citizenship (passport, birth certificate, or certificate of birth abroad)

• Residency in one of the 24 eligible U.S. states (check scholarship provider)

• Minimum cumulative GPA of 3.0 and good academic standing

• Acceptance to or enrollment at an accredited U.S. institution

Application Procedure:

1. Complete the Application Form – Fill out all required fields, use “N/A” where not applicable.

2. Personal Essay – 500–1,000 words covering:

   • Educational and professional goals

   • Impact of Cystic Fibrosis on life

   • Personal achievements or challenges

3. Three Letters of Recommendation – From a healthcare provider, school official, and community member

4. Required Documents:

   • Physician documentation confirming CF diagnosis
